By CNML Prep Editorial Team

Key takeaways
- The CNML exam has 115 multiple-choice questions (100 scored, 15 unscored pretest items) and you have 2 hours to finish.
- You need an RN license, a bachelor’s degree or higher with at least one nursing degree, and 2,080 hours as a nurse manager (or 4,160 hours in a leadership support role).
- The fee is $300 for AONL members and $425 for nonmembers. You test with PSI at a test center or by live remote proctoring.
- Leadership and Communication and Relationship Building carry 25 scored items each, half of the exam.
The CNML® (Certified Nurse Manager and Leader) is the certification for nurse managers and unit leaders. It is run by the AONL Credentialing Center, part of the American Organization for Nursing Leadership (AONL), and it is valid for three years.
This guide covers how the exam works, who can take it, what it costs and what it tests, based on AONL’s CNML handbook (revised December 2023) and AONL’s CNML page.

Format and timing

The exam has 115 questions and you have two hours, which works out to just over a minute per question. A short tutorial on the testing software comes first and does not count toward the two hours.
Fifteen of the 115 questions are pretest items AONL is trying out for future exams. They are mixed in with the others and do not count toward your score, so treat every question as if it counts.
You can skip a question, bookmark it and come back to it, and you can change an answer as often as you like. There is no penalty for guessing, so never leave a question blank. There are no scheduled breaks.
AONL’s handbook is clear that the questions are job-related and test application and analysis, not recall of isolated facts. Expect short scenarios that ask what a nurse manager should do first, or which approach is most effective.
Some questions need arithmetic, such as staffing or census math. You may bring a silent, nonprogrammable calculator without a paper tape or alpha keypad; phones and tablets are not allowed.
Who can take the CNML exam
You need all three of these:
- License: a valid, unrestricted registered nurse (RN) license. Nurses licensed outside the US verify their credentials through CGFNS International.
- Education: a bachelor’s degree or higher, with at least one degree in nursing from an accredited institution.
- Experience, one of:
- at least 2,080 hours in a nurse manager or primary unit leader role, or
- at least 4,160 hours in a comprehensive nursing leadership support role.
2,080 hours is one year of full-time work. The experience has to match AONL’s definition of a nurse manager: someone with clinical and administrative responsibility for their unit, including finances, human resources, staffing, performance improvement, technology and strategy.
AONL audits 10% of applications each year. If yours is picked, you will need your license verification, a transcript or diploma, and a letter from your employer confirming your hours.
Cost
| AONL member | Nonmember | |
|---|---|---|
| Exam fee | $300 | $425 |
Fees are not refundable. You can reschedule once for free by calling PSI at least two business days before your appointment; a second reschedule costs $100. If you do not test within your 90-day window, you lose the fee and have to apply again.
What the exam covers
AONL’s content outline divides the 100 scored questions into five domains:
| Domain | Scored items | Share |
|---|---|---|
| Communication and Relationship Building | 25 | 25% |
| Health Care Environment and Clinical Principles | 18 | 18% |
| Leadership | 25 | 25% |
| Professionalism | 14 | 14% |
| Business Skills and Principles | 18 | 18% |
Communication and Relationship Building covers relationship management, conflict, communication styles, building trust and influencing others: coaching staff and gaining consensus from stakeholders.
Health Care Environment and Clinical Principles covers care delivery models, healthcare economics and policy, regulation and accreditation, evidence-based practice, and patient safety and quality, including risk tools such as FMEA and root cause analysis.
Leadership covers systems thinking, change management (Kotter, Lewin, PDSA and Lean), diversity, equity and inclusion, decision making and delegation, professional governance, and innovation.
Professionalism covers professional and organizational accountability, health equity and social determinants of health, and working with the board.
Business Skills and Principles covers financial management (budgets, reimbursement, variances), strategic management (high reliability, goals, contracts, technology) and human resource management (recruiting, onboarding, a healthy work environment).
The CNML glossary defines the 50 terms that come up most across these domains.
Scoring
The passing score is set with the Angoff method: a panel of experts estimates how likely a minimally qualified nurse manager is to answer each question correctly, and those estimates set the number of correct answers needed to pass. When a new version of the exam is introduced, AONL uses a statistical process called equating so the standard stays the same across versions.
If you test at a PSI test center, you get your score report on screen. It shows pass or fail, plus your raw score (number correct) in each content area. AONL does not publish the passing score as a number or a percentage.
If you pass, AONL mails your certificate and pin in about four to six weeks. You can use the CNML® credential after your name once you receive written notification.
Retakes
If you do not pass, you can retake the exam no sooner than 90 days after your last attempt, and no more than four times in two years. Each attempt needs a new application and the full fee.
Recertification
The CNML is valid for three years. To renew, either:
- complete 45 hours of eligible professional development over the three years and pay the recertification fee, or
- pass the exam again within the year before your certification expires.
Eligible activities include academic courses, online courses, professional conferences and AONL self-assessment exams. You can submit your renewal up to a year before expiry. Late renewal, up to 30 days after the expiry date, costs an extra $50.
How to prepare
Start with the content outline, flag the tasks you do not do every week, and study those first. Then switch to practice questions as early as you can, because the exam tests how you apply what you know. Our CNML study plan lays out eight weeks, and the free CNML practice test shows where you stand today.
